Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi arrived at the facility unannounced to conduct a required annual visit. At 10:30 a.m., the LPA arrived at the facility and spoke with the Licensee, Floro Cortes Jr and explained the reason for the visit. At 11:05 a.m., the Licensee arrived at the facility. At 11:45 a.m., the Administrator, Nicole Cortes arrived at the facility. The facility is vendored by Tri-Counties Regional Center as a level 2 home.
Starting at 11:05 a.m., the LPA, along with the Licensee toured the physical plant areas inside and outside to ensure there are no health and safety hazards. Facility is a single-story residence that consists of four (4) client bedrooms, and two (2) bathrooms.
KITCHEN: The LPA observed the kitchen and dining area. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. At 11:08 a.m., hot water temperature measured at 108.4-degree Fahrenheit. Knives are stored and inaccessible to clients in the locked garage. Cleaning solutions, and chemicals are inaccessible and locked away in a cabinet.
BEDROOMS: The LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level.
RESTROOMS: Client restrooms are relatively clean and sanitary and in operating condition. At 11:21 a.m., hot water temperature measured at 106.4-degree Fahrenheit. The sinks had sufficient liquid soap, and paper towels. Continued on LIC 809-C.
