The inspector’s account

Licensing Program Analyst (LPA) Martha Arroyo conducted an unannounced Case Management - Incident visit to follow up on two (2) separate incidents received by the department on 05/14/2025 and 05/26/2026. Upon arrival, the LPA met with Program Director (PD), Tammy Worley and Day Program Supervisor (DPS), Tracy McCallon and explained the reason for the visit. Entrance interview conducted.

The first report from 05/14/2026, stated that staff was sleeping while assigned to a group of two individuals. When staff was asked about them sleeping, staff stated, “only asleep for 5 minutes”. Staff was placed on Administrative leave pending an investigation. Individuals present in staff’s care were checked on and reassigned after stating they were doing good.

The second report from 05/26/2026, stated that while a staff was in Program Manager's office, they heard Staff #1 (S1) speak to Individual #1 (I1), "You need to stop. You need to stop being bad" The tone and verbiage used caught the staff’s attention, so they immediately got up and walked out to observe the situation. I1 was seeking another cup of decaffeinated coffee and S1 was trying to help the staff assigned to I1 get them back to their group to continue to work on their goals.

During today’s visit, the LPA conducted a plant tour to ensure there are no health and safety hazards, interviewed seven staff and three individuals, conducted a file review, and obtained copies of pertinent documents relevant to the investigation.

The LPA will return at a later date to complete the investigation if warranted.
