Complaint Investigation Report
Regarding the allegation, " Staff spoke inappropriately to client in care" it is the concern of the reporting party (RP) that on 04/24/2026, Staff 1 (S1) allegedly told Client 1 (C1) "If you don't act right on the bus, I'm going to beat you up." To investigate the complaint the LPA conducted interviews and a file review. Interview with the Day Program's Director revealed that they were informed of the incident and spoke to S1, and S1 admitted to stating something along the lines of "if you don't have a good weekend and I get a report that you weren't good I am going to beat you up" to C1. The Program Director also revealed that S1 has since then been terminated due to the incident. A review of an Incident Report that was submitted to Tri-Counties Regional Center confirmed the incident. Phone interview with S1 revealed that S1 told C1, "if you're not nice to my bus drivers I am going to beat you up". S1 further revealed that they were joking and denied ever physically hitting any of the clients. Based on the information obtained, S1 admitted to speaking inappropriately to the client, the Department has sufficient evidence to support the allegation, therefore the allegation is Substantiated at this time.
Pursuant to Title 22, California Code of Regulations, the following deficiencies are cited (refer to LIC9099-D). Exit interview conducted, appeal rights discussed, and a copy of this report issued.
