The inspector’s account

Licensing Program Analyst (LPA) Esther Cortez conducted an unannounced Case Management-deficiencies visit to this facility in conjunction with a complaint. The purpose of this Case Management visit is to issue a citation for a deficiency observed during the course of the complaint investigation that is not directly related to the complaint allegation.

Upon arrival LPA briefly toured the Day Program and observed Client #1 (C1) sitting in the arts and crafts area with a gait belt on from 11:30 a.m. to 12:53 p.m. Per the DP's supervisor, C1 cannot self release the gait belt, and staff assist C1 to walk with the gait belt. The Supervisor did not know why the client had it on as they were sitting and not being assisted to walk at the time. File review revealed that C1 does not have a physician's order for a gait belt and DP does not have an approved postural support ex ceptions letter for C1.

Pursuant to Title 22, California Code of Regulations, the following deficiencies are cited (refer to LIC809-D). Exit interview conducted, appeal rights discussed, and a copy of this report issued.
