Facility Evaluation Report
Licensing Program Analyst (LPA) Teresa Camara arrived at the facility unannounced to conduct a required annual visit at 9:08 a.m. LPA met with administrator Lisa Lopez and explained the reason for the visit. There are thirty-one (31) clients physically at program, seven (7) clients doing remote activities and a total of eighteen staff (18), of which some will be doing the remote learning with clients and some take clients out to do community activities. Common Activity Space : The facility is in an industrial park and is single-story. There are four fire extinguishers in the facility; all last serviced 3/20/2023. The fire suppression system was last tested 12/22/2022 by Elite Fire Protection; it is tested annually. The carbon monoxide detector was last tested 10/27/2023. There are several activity rooms, including but not limited to arts & crafts room, game room, computer room, garden room, music room, fitness room, and resource center. There are four offices in the facility and a room used for isolating any sick clients until they can be taken home. LPA did not observe any obstructions or hazards. Activities: Activities are both designed for individuals and groups. The LPA observed staff working with clients. Food Service : The kitchen area was clean and in good condition. Clients bring their lunch; the facility provides snacks. The kitchen has a refrigerator, dishwasher, sink, microwave, and toaster oven. There is no range but staff use an electric skillet to cook things like eggs. Restrooms : There are four restrooms which were clean and sanitary. Water temperature measured at 115.3*F at 11:50 a.m. Files : All staff on the facility personnel report have been fingerprint cleared and associated to this facility. In the event the facility is short of staff, the facility also uses staff from sister facilities. All sister facility staff are also associated to this facility. LPA reviewed four (4) staff and four (4) client files; all were complete with required documents and training for staff. Infection Control: There are hand sanitizers throughout the facility. The facility has a sufficient amount of PPE and COVID tests. The facility's procedures as it pertains to infection control are adequate. Disaster Plan: The facility has a disaster plan and conducts monthly evacuation drills.
No deficiencies were observed. Exit interview conducted. A copy of the report was issued.
