Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi arrived unannounced to conduct a 1-year inspection at the facility. At 11:00 a.m., the LPA met with staff and explained the reason for the visit. At 11:20 a.m., the Licensee Bettina Brown arrived at the facility.
Starting at 11:15 a.m., the LPA, along with staff, toured the physical plant areas inside and outside to ensure there are no health and safety hazards. The facility is a double-story residence that consists of five (5) client bedrooms, one (1) office, two (2) staff rooms, three (3) client bathrooms and one (1) staff bathroom.
KITCHEN: The LPA observed the kitchen and dining area. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Hot water temperature measured within required range, 116.6 degree F. Kitchen knives are locked and inaccessible in the office. There are two refrigerators and an extra freezer for client use.
BEDROOMS: The LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level. RESTROOMS: Client restrooms are relatively clean and in operating condition. Hot water temperature measured within required range. The sink had sufficient liquid soap, and paper towels.
OUTDOOR SPACE: The LPA observed the back patio which has a covered outdoor area for client use. There are no bodies of water on the premises. The garage is detached from the house and remains inaccessible to the clients. Continued on LIC 809-C.
