Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi arrived at the facility unannounced to conduct a required annual visit. At 8:30 a.m., the LPA met with the Licensee/ Administrator Charito Ramirez and explained the reason for the visit.
Starting at 8:44 a.m., the LPA, along with the Licensee toured the physical plant areas inside and outside to ensure there are no health and safety hazards.
KITCHEN: The LPA observed the kitchen and dining area. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food stored in the kitchen and garage. At 8:45 a.m., hot water temperature measured at 108.1-degree Fahrenheit.
BEDROOMS: The LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level.
RESTROOMS: Client restroom is relatively clean and sanitary and in operating condition. At 8:55 a.m., hot water temperature measured at 117.3-degree Fahrenheit. The sinks had sufficient liquid soap, and paper towels.
OUTDOOR SPACE: The LPA observed the back patio which has a covered area equipped with furniture for client use. There are no bodies of water on the premises. There is one (1) side gate for emergencies.
