The inspector’s account

Licensing Program Analyst (LPA) Esther Cortez alongside Tri-Counties Regional Center Quality Assurance Specialist (QAS) Katy Robinson arrived at the facility unannounced to conduct a case management-incident visit at the Day Program (DP) regarding a self-reported incident that occurred on 05/30/2025. LPA met with Program Manager Juan Garcia and explained the reason for the visit.

It was reported that on 05/30/2025, the DP was experiencing a medical emergency, staff was trying to clear a path for paramedics, and Client 1 (C1) got up from their seat and alleged that a Volunteer (V1) hit them on their left arm, and V1 was placed on leave. It was further reported that staff present did not witnessed the alleged incident, and incident was reported to all required agencies.

During today's visit the LPA and QAS interviewed the Program Manager, V1, C1, and three (3) staff. Interviews revealed that the DP was experiencing a medical emergency on 05/30/25, staff and volunteer were trying to move participants and tables out of the way for paramedics and/or fire fighters. Staff did not witness any staff/volunteer hit C1. C1 did not recall the incident and stated they enjoy coming to the program and has no concerns. Furthermore, the agency that oversees the volunteer program at the DP, conducted an investigation and determined V1 did not act neither maliciously nor abusive toward C1, and V1 has returned to the DP.

No immediate health and safety concerns were observed during the visit. Exit interview conducted and copy of report provided.
