Facility Evaluation Report
Licensing Program Analyst (LPA) Esther Cortez conducted an unannounced Case Management – Deficiencies visit in conjunction with complaint control # 29-AS-20260629104137. The LPA met with Executive Director Rafael Silva and informed them of the reason for the visit.
During the investigation of complaint control #29-AS-20260629104137 , the LPA obtain information that on 06/15/26, Resident 1 (R1) complained of leg pain, had old bruising, had an appointment with their doctor, and was sent to the hospital. A Special Incident Report (SIR) was not submitted to Community Care Licensing as required. In addition, during interviews, staff admitted they are not aware of the mandated reporting requirements.
Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiency cited (refer to LIC 809-D):
