Facility Evaluation Report
Licensing Program Analysts (LPAs) Kelly Dulek and Teresa Camara arrived at the facility unannounced to conduct a required annual visit. Upon arrival, the LPAs met with Facility Designee Andrea Davis and the reason for the visit was explained. Entrance interview conducted.
Beginning at 10:56AM, the LPAs, along with Facility Designee and Resident Care Director Maria Gomez toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations. There are four (4) resident buildings in total. Two (2) Assisted Living (Maricopa and Sespe) and two (2) Memory Care (Topa Topa and Matilija). The following was noted:
OUTDOOR SPACE: The facility has an in-ground pool, which was observed to be fenced and locked in compliance with regulation. Memory Care courtyard does have a fountain. Staff stated residents are supervised when in proximity to the water feature. The outdoor fireplace in the main courtyard was noted to be adequately screened. LPAs noted that the 3 (three) exit gates from the memory care unit out to the parking lot and/or the main outdoor courtyard were locked with a key. LPA spoke with Ventura County Fire Department (VCFD) Fire Inspector during today's visit and clarified that 2 (two) egresses are required to remain clear at all times in each outdoor area and the facility cannot be cleared for a locked perimeter due to the layout.
KITCHEN: Each building has their own kitchen and dining area. The LPAs inspected the kitchen/food service area at all four buildings. Kitchen appliances were in operable condition.
