Facility Evaluation Report
Licensing Program Analyst (LPA) Teresa Camara conducted a Case Management - Incident visit regarding a self-reported incident which took place on 12/2/2024. LPA was joined by Tri-Counties Regional Center Quality Assurance Specialist (QAS) Liz Aced-Arnett. LPA and QAS met with program manager Amber Lundeen and explained the reason for the visit.
On 12/2/2024, Client 1 (C1) alleged to a case manager that Staff 1 (S1) pushed and hit them. On 12/3/2024, C1 alleged to the program manager that S1 pushed them and twisted their hand. During today's visit, LPA and QAS interviewed the program manager starting at 9:50 a.m., interviewed S1 at 10:16 a.m. and interviewed C1 at 10:25 a.m.
The program manager stated they conducted their own investigation. They were concerned about C1 changing stories about what happened. C1 has had behavioral issues regarding boundaries, especially with female staff, clients and women in the community. C1's family has requested C1 not be allowed to leave the facility to return home until after 1:30 p.m. each day. C1 rides their bike to and from this day program. If C1 leaves early, it is typically S1 who calls C1's brother to report this which upsets C1.
There were no witnesses to what C1 alleged. C1 confirmed there were no marks or bruises on their body from the alleged kick/shove/twist of hand. S1 denied ever being alone with C1. S1 does administrative duties and works with groups. S1 is never alone with any clients and noted C1 is mostly independent, moving around the facility with their friends.
The program manager stated S1 was put on a two day suspension while they conducted their investigation. A Crisis Prevention (CPI) refresher class has been conducted on 12/4/24 and a make up class for staff who were absent will be conducted 12/9/24.
Based on information obtained in interviews, there does not appear to be evidence of abuse by S1. No deficiencies observed. Exit interview conducted and report issued.
