The inspector’s account

Licensing Program Analyst (LPA) Zabel Chochian conducted and case management visit today. Upon arrival LPA met with staff and later with Cami Hershkovitz. Reason for the visit was explained.

On March 15, 2024, Cami Hershkovitz applied for renewal of her administrator certification. However, did not complete required additional courses and falsified certificates of completion. Application for certification was denied. Ms. Hershkovitz appealed the denial.

Per Decision and Order/ CDSS Case No. 8025001001 /OAH No. 2025050993; on 8/11/2025 - Cami Hershkovitz’s application for administrator certification was denied.

The following deficiency is cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code.

Exit interview conducted. A copy of the report and appeal rights were provided.
