Facility Evaluation Report
On 05/14/2026, an Informal Conference was held at the Woodland Hills Adult and Senior Care Regional Office. In attendance included Licensing Program Manager (LPM) Desaree Perera, Licensing Program Analyst (LPA) Martha Arroyo, and Facility’s District Manager Joanna Iniguez and Quality Assurance Service (QAS) Nelson Rivera.
The purpose of today’s Informal Conference is to discuss a complaint dated 01/30/2023 (Complaint Control # 29-AS-20230130113402). The Administrative Action process was explained to the licensee, as well as the role of an Informal Conference.
On 01/30/2023, the Department received a complaint alleging that a client was being sexually abused by another client while under the care and supervision of the facility. The complaint was substantiated on 05/09/2023; however, after receiving additional information the allegations were determined to be unsubstantiated, therefore an amended report was issued on 05/29/2024. Nonetheless, on 05/29/2024, the Department conducted a Case Management – Deficiencies visit and issued citations for violations observed during the complaint investigation that were not alleged in the complaint. These citations included Reporting Requirements and Needs and Services Plan.
During today’s meeting discussions were held regarding the importance of staff reporting possible abuse and the importance of submitting incident reports and Suspected Dependent Adult Abuse (SOC341) timely.
