The inspector’s account

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit for the purpose of investigating self-reported incident report. LPA was joined by Tri-Counties Regional Center (TCRC) Quality Assurance Specialist (QAS) Katy Robison. Upon arrival LPA and QAS met with staff and explained the reason for the visit. District Manager Joanna Iniguez arrived shortly after.

On 06/11/2025, the Regional Office received a self-reported incident report which stated, that the Administrator allegedly grabbed Resident #1 (R1), by the collar and physically dragged R1 into R1's room. The report continued to state that the Administrator was heard using offensive language towards R1 and administrating medication that was not prescribed to R1. No dates or details of medication were provided to facility staff who submitted incident report.

At approx 09:40am. LPA and QAS conducted physical plant, interviewed staff, client and reviewed and obtained copies of pertinent documentation relevant to the investigation.

Based on interviews and records review no immediate or potential health and safety concerns were observed during the visit and no deficiencies cited at this time. LPA and QAS has determined should further investigation be warranted, LPA and QAS will return at a later date.

Exit interview conducted. A copy of the report was issued.
