Facility Evaluation Report
Licensing Program Analyst (LPA) Kelly Dulek conducted an unannounced Case Management – Incident visit at 01:24 PM. LPA met with Program Administrator Deleene Farquharson and explained the reason for the visit. Entrance interview conducted.
The Department received an email on 03/26/2026 indicating that Client #1 (C1) had passed away due to a choking incident. LPA Brian Balisi called the facility and requested additional information from the facility Administrator.
During today’s visit, LPA Dulek spoke with Program Administrator, reviewed C1's file and obtained copies of relevant information, obtained a copy of the unusual incident report, and LPA toured the facility with Keegan Hornbeck, LCSW. LPA requested Program Administrator submit an LIC 624A, Death Report via email to the Regional Office. Additional information from the Medical Examiner's Office is pending at this time.
No immediate health and safety concerns were identified during today's visit. If further investigation is needed, a Department representative will return at a later date.
