Facility Evaluation Report

Licensing Program Analyst (LPA) Valeria Conway conducted an announced Case Management - Other visit to the above facility. The LPA met with Licensee, Teresita and Larry Wayne as the purpose of the visit was to conduct a final walk-through of the facility prior to closure. Entrance interview conducted.

At approximately 8:40 A.M. LPA gained entry to the facility and conducted a physical plant tour with the Licensee(S). During the walk-through, LPA observed the facility to be vacant and verified that no care and supervision services were being provided.

The licensees reported that plans to close the facility began in October 2025; however, closure was delayed due to the need to relocate one remaining resident. The resident was successfully relocated on March 11, 2026, however, their belongings stayed behind. On June 2, 2026, LPA received confirmation that the resident’s personal belongings were delivered on May 2, 2026.

As of June 3, 2026 facility #565800906, Golden Year Care, located at 1325 Lantana St. Camarillo, CA 93010 is closed and no longer in operation. The physical copy of the license was surrendered to the LPA.

Closure of the facility is confirmed as of this visit. Exit interview conducted. A copy of the report was provided.
