Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo conducted an unannounced Case Management - Incident visit to follow up on an incident report received by the department on 04/30/2025. Upon arrival, the LPA met with staff. The Clinical Director, Tiffany Curtis arrived shortly after and the reason for the visit was explained. Entrance interview conducted.

It was reported that on 04/02/2025, Client #1 (C1) signed out at 3:17 p.m., but had not returned after 10:00 p.m. curfew, had not called the facility to check in or returned after midnight. On 04/04/2025, Vista Voice contacted facility stating C1 was at their location but did not want to go back to the facility. The facility contacted the police department to report C1 as missing. C1 returned to the facility on 04/09/2025 at approximately 7:00 a.m. after being dropped off by law enforcement.

During today’s visit, between 9:45 a.m. and 10:30 a.m., the LPA conducted interviews with one staff, a file review, and obtained copies of pertinent documents.
