Facility Evaluation Report
Licensing Program Analyst (LPA) Valeria Conway conducted an unannounced Case Management Deficiency visit in conjunction with an initial 10-day complaint visit (CC #29-AS-20251020160401 ). LPA met with interim Executive Director (ED), Remon Pagels. The purpose of this report is to issue citations for deficiencies observed during the complaint investigation which were not related to the complaint.
Woodland Hills Regional Office received an email on 10/26/2025, notifying that a chang e of Administrator/ED for Atria Las Posas was effective 10/11/2025. The email included attachments containing the new interim ED's information and the required supporting documents.
During today's visit, LPA reviewed the Guardian System, which reflected that the new Administrator is fingerprint cleared; however, they are not associated with the facility as of today.
An immediate civil penalty of $500 for back ground check - Association transfer violation is assessed today. Administrator was informed that additional civil penalties might be assessed based on Health and Safety Code 1569.49(e) and 1569.49(f).
