The inspector’s account

Licensing Program Analysts (LPAs) Valeria Conway and Kelly Dulek conducted a Case Management - Deficiencies visit in conjunction with a complaint visit conducted on today's date. LPAs met with facility designee, Agnes Gazaryan. The purpose of this report is to issue citations for a deficiency observed during the complaint investigation.

During facility tour, which began at 12:14PM, LPAs observed that the fire pull station in the 200s hallway is non-functional and is taped into position. Interview revealed differing time frames that the fire pull has been non-functional, however the minimum reported time frame is a week without repair. LPA called and spoke with the Fire Inspector at 02:20PM, who confirmed that all fire protection systems need to remain in place at all times and since the pull station was engaged and is currently taped up, the facility is out of compliance.

Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiency was cited (refer to LIC 809-D.) A civil penalty was issued in the amount of $1000, as it is a repeat citation. Facility Designee was informed that failure to correct the deficiencies may result in additional civil penalties.

Exit interview conducted, appeal rights discussed, and a copy of this report and appeal rights were provided.
