Licensing Program Analysts (LPA) Valeria Conway conducted an unannounced Case Management Visit - Other to the facility above. At 11:30 A.M. LPA met with Marketing Director, Ashley Kumar.
Licensee and Back-up administrator were in a meeting at the time of LPA arrival. After the meeting LPA explained the reason for the visit. The licensee authorized representatives to sign today's report. Entrance interview conducted.
The purpose of today's visit was to obtain the missing signatures from the visit conducted on Friday 10/24/2025.
