Facility Evaluation Report

At 9:35 A.M. Licensing Program Analyst (LPA) Valeria Conway conducted an unannounced case management visit to offer Executive Director (ED), Monica Reyes, the opportunity to be referred to the Technical Support Program (TSP), as previously mentioned during prior visits. LPA met with ED and explained the purpose of the visit.

A physical plant tour was conducted to ensure there are no health and safety concerns.

During today’s visit, the LPA encouraged the ED to consider participating in the TSP. This is a non-enforcement program offered by the Community Care Licensing Division (CCLD) that provides services and resources to assist providers in meeting licensing requirements. The TSP is designed to help facilities achieve and sustain compliance through training, support, and access to relevant tools.

LPA provided the ED with the following contact information for TSP services. TSP email address TechnicalSupportProgram@dss.ca.gov and their phone number (916) 654-1549. Additionally, LPA advised the ED to review the Provider Information Notices (PINs) on CCLD's website ( www.ccld.ca.gov ) for further guidance and updates.

ED agreed to contact TSP for further information and assistance.
