Facility Evaluation Report

Licensing Program Analysts (LPAs) Kelly Dulek and Valeria Conway conducted a case management visit related to a self-reported incident report. LPAs arrived at the facility at 10:14AM and met with Executive Director (ED) Monica Reyes shortly after the visit began. Entrance interview conducted.

An incident report was received at the Woodland Hills Regional Office on 05/19/2025. Incident report indicated that on 05/17/2025, at approximately 06:30PM, staff observed Resident #1 (R1) in their room on their cell phone. R1 was observed with "excessive blood dripping" from R1's left arm. Additional staff were called for assistance. Facility staff attempted but were unable to stop the bleeding. 9-1-1 was called and R1 was transported to the hospital. Report indicates that the resident passed away in the hospital.

During today's visit, LPAs interviewed ED, toured the facility with ED at 10:47AM, reviewed R1's file and obtained documents relevant to the investigation.

Executive Director was informed that either LPA or Community Care Licensing Division (CCLD) Investigations Branch (IB) investigator will follow up related to the self-reported incident.
