Facility Evaluation Report

Licensing Program Analyst (LPA) Steve Chang conducted an unannounced Case Management visit today, and met with Executive Director (ED) Shantela Yadao.

This visit is following up with the case management on 8/11/2022.

ED already provided the training on resident rights to all staff.

All the staff also registered in the training of redirecting dementia residents with behaviors in August 2022.

Exit interview was provided to ED. This report was provided to ED for signature. A copy of this report was provided to ED.
