Facility Evaluation Report
On May 21, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced case management visit to deliver a finding that was observed during the complaint investigation. LPA met with the administrator and explained the purpose of today's visit.
During the investigation of complaint # 26-AS-20260330133147, it was observed that facility did not report to CCL of an incident that happened on March 20, 2026 concerning a resident (R1) who left the facility unattended. The administrator acknowledged that it was not reported to CCL in a timely fashion.
Based on observation interview and record review, deficiency is cited under California Code of Regulations, Title, 22 cited on the LIC 809D. Failure to correct the deficiencies may result in civil penalties.
This report is reviewed and discussed with the administrator.
