Licensing Program Analyst (LPA) Christine Dolores arrived at the facility unannounced to conduct a Case Management - Other visit. LPA met with Executive Director, Diane Atkinson.
The purpose of the visit was to deliver an immediate exclusion letter to exclude an employee (S1) at the facility.
The immediate exclusion letter and declaration of service was handed to the Executive Director during today's visit.
This report was reviewed with Executive Director, Diane Atkinson and a copy of the report was provided.
