On 8/15/25 LPA Grace Donato made an unannounced annual visit to the facility. LPA met with Administrator Elaine Johnson. LPA explained the purpose of the visit.
Facility is currently under construction. Residents have been moved to a different facility prior to construction starting. There are no resident & staff records to review.
Administrator will keep the LPA informed regarding the progress of the construction.
No deficiencies are cited at this time. Report is reviewed and a copy is provided.
