On 5/9/2025 LPA Grace Donato made an unannounced case management-other visit. LPA met with Administrator Elaine Johnson. LPA explained the purpose of the visit.
LPA visited the facility to conduct a check regarding a relocation notice for residents due to facility renovation. All rooms have been emptied, and all residents have been moved to a different facility.
After renovations are completed, residents will be moved back.
Licensee will be updating LPA before residents move back in.
