Facility Evaluation Report
On 12/5/2025, LPA Grace Donato conducted a Case Management - Incident visit at the facility. LPA met with Program Director Ebony Foy and explained the purpose of the visit.
LPA conducted a tour of the facility, interior and exterior to ensure there is no potential or immediate health and safety risk at the facility.
On 03/11/2025, the Department received an incident report indicating staff member (S1) sexually abusing client (C1) while in care. It was revealed that, on 03/11/2025, a library door was shut and was blocked by something heavy. Staff member (S2) was unable to push open the door. When S2 was finally able to push open the door, S1 was sweating and C1 was pulling up the pants. Results from evidence submitted revealed S1’s bodily fluid was found in C1’s diaper. S1 was arrested and charged with Penal Code (PC).
Based on the review conducted by the Department and information gathered, the following deficiency was cited on LIC 809-D per the California Code of Regulations (CCR), Title 22, Division 6, Chapter 3.
