Facility Evaluation Report

On May 7, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced case management visit to follow up on an incident that was report by resident #1 (R1)'s facility/residence. LPA met with the Regional Director and explained the purpose of today's visit.

On April 29. 2025, FHAR- Sneath Lane Home where R1 resides reported that on 4/28/2025 at 11:30PM, R1 was observed by facility manager standing next resident #2 (R2)'s bed, inappropriately dressed (R1 and R2 are roommates) and subsequently scurrying away. Staff did not observed any changes with R2 and the facility implemented safety measures to ensure the safety of both residents.

During today's visit, LPA was not able to observe R1 as R1 is doing community activities with staff members. However, the Regional Director reported that there has no reports of R1 exhibiting any unusual behaviors at the day program.

The Regional Director provided resources to LPA to obtain additional details.

This report is reviewed and discussed with the Regional Director.
