On 3/12/2026, LPA Grace Donato conducted a Case Management - Incident visit at the facility. LPA met with Program Director Ebony Foy and explained the purpose of the visit.

LPA confirmed that facility has received the Decision & Order for S1. S1 has not been at the facility since 3/12/2025 and ceased employment on 4/10/2025.
