The inspector’s account

On 7/9/2026, LPA Grace Donato made an unannounced post-licensing visit to the facility. LPA met with Administrators Bill Fajarado and Claire Ascalon. LPA explained the purpose of the visit.

LPA toured the facility. The indoor and outdoor passageways were free of obstruction. One client has just been admitted to the facility.

Resident records are updated, complete and signed. Staff records are complete.
