Facility Evaluation Report

On 4/3/2025, LPA Grace Donato conducted an unannounced collateral visit at Sonesta Hotel located in South San Francisco, CA. LPA met with Administrator, Editha Guevarra and explained the purpose of the visit.

This visit in in response to facility relocation due to landlord ending the lease abruptly. New facility is currently undergoing renovations.

During the visit at the hotel, LPA observed 2 hotel rooms housing 2 clients in each room. Clients rooms were observed with clients' personal belongings, water, snacks, bathroom, towels, bed, and a kitchenette.

According to the administrator, one client does day program. No staff members will be sleeping in the clients' rooms as all staff are wake-staff. Administrator indicated they will provide breakfast to clients; cereal, fruits and eggs with milk, coffee or juice and for lunch and dinner they will have take-out.

According to administrator, the tentative check out date from the hotel is on 4/19/2025 in the morning and clients will move to new facility address. Administrator will notify LPA when all clients have been moved to the facility.

No citations are issued during the visit. Report is reviewed and a copy is provided.
