Facility Evaluation Report
On 7/8/2026, LPA Grace Donato conducted an unannounced Case Management - Incident. LPA met with House Manager Crisel Onate and Administrator Lilibeth Rapadas followed after. LPA explained the purpose of the visit.
Regarding the incident report received 7/6/2026. It was reported that a resident (R1) had become agitated and attempted to hit another resident. R1 was given medication but the agitation persisted and R1 started hitting a staff member (S1). 911 was called and police assisted.
During the visit LPA observed R1 to be in good mood and was helping with sorting out his/her clothing with the staff and assisting in other chores. There were no injuries found on R1.
Staff is aware of the behaviors and are doing redirection if the behaviors show. A meeting with the doctors have been scheduled and facility will update R1s plan of care.
No deficiencies for today. Report is reviewed and copy is provided.
