On 4/10/2026,LPA Grace Donato arrived at the facility to complete the Annual 1-year required inspection. LPA met with House Manager Ramon De Los Santos and explained the purpose of the visit.
LPA reviewed three resident files and three staff files and all are complete and updated. Medication review was done, and all medications are accounted for, and centrally stored medication records are updated. Emergency drills are logged and done every month.
No deficiencies were cited during today's visit.
