The inspector’s account

On 6/16/2025 Licensing Program Analyst (LPA) Murial Han conducted an unannounced visit. LPA met with caregiver, Mark Katigbak and LPA explained the purpose of today's visit.

LPA delivered an immediate exclusion letter to exclude a staff (S1) who is no longer working at the facility.

The letter was provided to the caregiver and the facility Manager, Ramon Delos Santos was informed over the phone.

This report is reviewed and discussed with caregiver and a copy is provided.
