Facility Evaluation Report

As a follow up to annual inspection of 3/28/25, LPA Jeung met with administrator to review facility's cash handling procedures--including record keeping--as per citation issued for Section 80026 Safeguards for Cash REsources.

LPA Jeung also discussed the following with administrator:

- Annual licensing renewal fee of $454 is overdue

- Identification of rooms is inconsistent with facility sketch initially submitted, and must be in accordance with room approved by fire department as non-ambulatory (rear bedroom)

- Bank statement for clients' personal and incidental money is requested with explanation of facility's cash handling and record keeping procedures

- Written request to correct mailing address to be submitted, signed by corporation CEO or president
