On October 31, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management-Other visit. LPA met administrator, Victoria Flores and explained the purpose of today's visit.
LPA delivered an immediate exclusion letter for a staff #1 (S1) who is currently associated in the facility. The administrator was advised that S1 is not allowed in the facility.
The exclusion letter was provided to the administrator.
This report is reviewed and discussed, and a copy is provided.
