Facility Evaluation Report

On 10/23/2025, Licensing Program Analyst (LPA) Vado Jaime Vado conducted an unannounced case management - incident visit in response to an incident report received on 10/15/2025 regarding a client elopement. LPA met with assistant administrator Jhaynel Patdu and explained the purpose of today's visit. There are no residents present in the facility at this time as they are all in day programs.

Per the incident report received, C1 was observed to be missing from their room at approximately 7:30am. Upon discovery staff searched for the resident and the client was not located despite staff searching the area and neighborhood. Client was found around 8 to 9 hours later, around 5pm, at a county park located approximately 2 miles away. Client was unharmed and returned to the facility unharmed. Per staff and file review, the client is not able to be left unassisted on their own outside of the facility.

This poses an immediate health and safety risk for the client in care. A citation is issued on this day on the following LIC809D.

Report is reviewed with Jhaynel and copy is provided.
