The inspector’s account

On 08/21/2025, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced required 1 year annual inspection visit. LPA met with administrator Jennifer Octabiano and explained the purpose of today's visit.

This is a single level facility and licensed to serve age range of 18 and over, 32 clients, all of which may be non-ambulatory. During today's visit there are 21 clients attending the program today, 1 is attending remotely. The physical plant was toured inside and outside of the facility to ensure the safety of the clients. There are no cameras present in the facility. LPA observed the facility kitchen which is clean and observed appliances that are in good repair. Facility does not provide food besides water and small snacks. Clients bring their own lunches into the program. LPA observed organized shelving holding client lunch boxes which are labeled. No knives are present in the kitchen per observations made. First aid kit is observed as complete with required items. This program does not handle or hold any medications for clients. There is a quiet that does have a medication cart but it is empty and locked. First aid kit is housed in the quiet room. LPA observed that there are multiple fire extinguishers in place last inspected on 09/04/2024, smoke detectors, carbon monoxide detectors are observed in place through out the facility, and central HVAC. Facility is not equipped with fire sprinklers. PPE is observed to be in place and staff are wearing masks during today's visit. Emergency exit routes are observed inside and outside to be free and clear of obstructions. Last emergency/disaster drill was conducted on 07/21/2025 which was a fire drill followed by discussions regarding earthquakes per documentation reviewed. Water temperature is being tracked via a log observed posted on the refrigerator in the kitchen. Water temperature was measured at 111F tested on this day by LPA in the mens restroom. Water temperature is tested every morning according to the log observed. Cleaning supplies and toxins are observed to be locked in a storage closet in the exercise room.
