Facility Evaluation Report

To follow up on an ARF application that was to be submitted to Central Applications Bureau (CAB) at 744 P Street, MS 9-14-8201, Sacramento 95814, as discussed on 5/5/25 by LPA Jeung and applicant Brian Galicia, LPA Jeung met with Brian and Maricel Galicia. Proof of payment and delivery via USPS to a mail room in zipcode 95800 on 5/13/25 is obtained. However, as per CAB, no application for the facility address has been received. LPA advised Mr. Galicia to follow up and file a claim of non-receipt with U.S. Postal Service.

Based on information obtained by LPA--which includes signed "contract of sale" dated May 2022 between seller Maria Baris and buyers Nelson, Brian and Maricel Galicia--there was a 100% transfer of stocks/shares of LJB Corporation, licensee. Deficiency of the California Code of REgulations, Title 22, is cited on a following page.
