Facility Evaluation Report
On 4/21/2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct the Annual 1-year required inspection. LPA Calandra was greeted by Rachel Buhay, Clinical Lead and Dayana Quesada, Direct Support Professional(DSP) and explained the purpose of the visit.
LPA reviewed resident files. Based on record review, R1 does not have a medical assessment with Ambulatory status. A Type B citation was issued.
Deficiencies are cited under California Code of Regulations(CCR). Failure to correct said deficiencies by the Plan of Correction due date may result in Civil Penalties.
The Annual inspection will be completed at a later date.
An exit interview was conducted. A copy of the report along with Appeal Rights were provided to the facility representative.
