On May 28, 2026, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit to deliver an Amended report from 03/19/2026. LPA Calandra was greeted by Licensee, Krizia Caliwag-Boyer and explained the purpose of the visit.
LPA explained the purpose of the Amended Report and showed Licensee, Krizia Caliwag-Boyer the amendment. A copy of the Amended report was provided to the Licensee.
No deficiencies cited during today's visit.
An exit interview was conducted and a copy of the report provided to the Licensee.
