Facility Evaluation Report
On 7/24/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to an incident that occurred on 7/12/2025 in which a resident missed their evening medication dose. LPA Calandra was greeted by Sally Dela Cueva, Administrator and explained the purpose of the visit.
Based on interview, S1 was administering medication to R1 when R2 approached S1 and asked for assistance. S1 locked R1's medication in the medication cabinet and went to assist R2. The next day on 7/13/2025, the Administrator discovered that S1 had forgotten to provide R1's medication to them the night before. This is a Type A deficiency and a citation was provided.
Deficiencies are cited under the California Code of Regulations, Title 22. Failure to correct the deficiencies by the due date may result in civil penalties.
An exit interview was conducted. This report was reviewed with facility representative and a copy of the report along with Appeal Rights was provided.
