Facility Evaluation Report
On 1/6/2025, Licensing Program Analyst(LPA) John Calandra arrived at the facility to conduct a Case Management visit in regards to an incident that was reported to the Department involving a staff member and a client. LPA Calandra was greeted by Richard Morales, Direct Support Professional and explained the purpose of the visit. Sally Dela Cueva, Administrator
According to the Administrator someone reported to them that R1 had been potentially injured by S1. LPA Calandra will be returning to the facility at a later date to follow up on the incident as R1 had just left for a medical appointment when the LPA arrived at the facility.
No deficiencies cited during today's visit. An exit interview was conducted and a copy of the report provided.
