The inspector’s account

On June 29, 2026, Licensing Program Analyst (LPA) Murial Han conducted an unannounced annual inspection. LPA met with co-administrator, Kaila Branch and LPA explained the purpose of today's visit.

LPA toured facility and grounds. This 4- bedroom home has 2 full bathrooms, kitchen, office, and living area, and accommodates personal with special health care needs. There is a rear deck and level, fenced back and side yards. A detached storage shed is in the backyard. There are no accessible bodies of water nor fire safety hazards observed. In each clients' private bedroom, there is a sink and locked medication cart. LPA observed toxins, sharps, and chemicals are locked and inaccessible to residents in care.

LPA observed the facility is clean, safe, sanitary and in good repair. Hot water temperature in the bathroom and resident bedroom sinks was measured at 108- 110 degrees F.

LPA observed liquid nutrition for 3 residents--and first-aid kit are inspected and complete.

A review of (4) resident files was conducted and noted on the LIC 858.

A review of (3) staff files was conducted and noted on the LIC 859.

During today's visit, there was no residents present as they are all attending day program.

LPA reviewed P & I for 4 residents to be adequate.

This report is reviewed and discussed with the co- administrator and a copy is provided.
