On September 11, 2025, Licensing Program Analyst (LPA) Grace Donato conducted an unannounced Case Management-Other visit. LPA met with Administrator, Bill Fajardo and explained the purpose of the visit.
LPA delivered an immediate exclusion letter for a staff member who is currently associated in the facility.
The letter was given to and reviewed by the Administrator.
