Facility Evaluation Report

On August 4, 2025, Licensing Program Analyst (LPA) Murial Han conducted an unannounced Case Management visit to follow up on an incident that was reported by the facility. Upon entry, LPA met with caregiver, Brenda Parco and explained the purpose of today's visit. The lead staff, Mariteresa Manuel arrived at the end of the visit and LPA explained the purpose of the visit.

On August 4, 2025, the facility reported that on July 30, 2025, staff observed resident #1 (R1)'s right hand was swollen and signs of bruising. The facility contacted R1's day program to obtain information related to the injury and asked facility staff and they reported that they did not observed anything that may have caused the injury.

During the visit today, LPA observed R1 was sitting in the living room and appeared to be pleasant and calm. LPA observed the top of R1's right hand to be swollen with some greenish and blackish discoloration.

During today's visit, LPA also collected documents.

This incident requires further investigation.

This report is reviewed and discussed with the lead staff.
