Facility Evaluation Report

On June 11, 2025, Licensing Program Analyst (LPA) Murial Han conducted a Case Management visit to follow - up on an incident that was reported by the facility. Upon entrance, LPA was greeted by the nurse, Joliza Tambot and LPA explained the purpose of today's visit. The House Manager, Veronica Cochon arrived shortly thereafter and assisted with the visit.

On June 9, 2025, the facility reported that on June 5, 2025 resident #1 (R1) presented physical aggression toward resident #2 (R2) and staff #1(S1) resulting injuries to S1 while attempting to calm R1. The facility called 911 and R1 was transferred to the hospital for further evaluation.

The facility reported that R1 stayed at the hospital for a few days and returned on 6/9/2025.

The facility reported that R1 met with the Psychiatrist yesterday and medications were adjusted.

The facility reported that R1 has been having escalated episodes of physical aggression toward other residents and facility staff for the past few months. Facility staff attempted to de-escalate in every incident but resulting staff injuries.

During today visit, LPA interviewed facility staff, collected documents and observed R1 who appeared to be loud with some agitation when interacting with staff but staff was able to redirect R1.
