Complaint Investigation Report

During the tour, LPAs observed the holes on the wall from the installed cameras and they were located in the kitchen, in the living/dinning room and in the hallway and LPA did not observed anything in resident's rooms and bathrooms.

After the investigation, this allegation is deemed to be unsubstantiated as LPAs observed cameras were once installed in the common areas and not in the resident's rooms and bathrooms and after the meeting with facility staff, the Licensee removed all the cameras.

Although the above allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegation is UNSUBSTANTIATED.

This report is reviewed and discussed with the manager. A copy is provided.
