On 02/12/2026, Licensing Program Analyst (LPA) Jaime Vado conducted an unannounced case management visit in order to deliver an amended LIC809 and LIC809C dated 11/05/2024. LPA met with Caregiver - Zehafta Yohannnes and explained the purpose of today's visit. Currently there is one client present and the caregiver.
During today's visit LPA explained to Zehafta the delivery of the amended document. LPA showed where the amendment was made and provided copies of this report and the amended report on this day.
This report is reviewed with Zehafta and a copy is provided.
